I had a medical history/records sheet done by Shirley Green at PRO several years before my Mother passed
away. With passing years the doctor and hospital visits became more frequent. As most people know, we are
not always lucky enough to have the same nurse or doctor at the hospital and sometimes not even in a doctor's
office.
Each time that the questions would start, about what medications she was on or if there had been any changes since the last visit, I would pull out the latest updated sheet that showed all her medications and any medical procedures that had been done. Every nurse I ever handed this to was ecstatic. They always commented on how they wished everyone had this information so readily available and correct. It got to the point with our regular doctors that before they even started they would ask if there was a new sheet. When we would end up in the emergency room at the hospital it helped expedite things greatly. It also answered any questions the doctor had about history or what he might be dealing with as a reaction.
Each of us three children had a copy of this sheet so no matter who was with Mom on any given day the information was updated and correct. Living out of town, I was not always up on the day to day matters of Mom's care. When my sister had a challenge with the timing of an appointment, and my brother or I would step in, it really came in handy.
I would dread to think of the things that could have happened, with medicine reactions and such, had we not had this available.
On a more personal level, I have given a copy of this history to my own family doctor to show part of the medical history I come from. This has helped greatly with knowing what we need to watch out for or what I may be at high risk of.
I believe everyone should have this type of record kept on every family member, young and old alike. A traveling teenager or an elderly parent can have a medical issue at any time. In the case of day to day medical care it is so helpful and in the case of an emergency, it's time and life saving! Sincerely,

Invaluable for speeding up frequently required medical service in-processing, for my elderly Mother.
The medical professionals we dealt with routinely began to ask us for the latest 'Pink Sheet' when they'd see us bringing in our elderly Mother for care.
Use the 'Pink Sheet' to answer those endless technical questions the Health Care Providers need to have answered in order to treat you or your loved one.
Provides a margin of safety in that medical procedures and medications/dosages are correctly spelled and in the correct chronological order, minimizing errors from miscommunications and/or forgotten information.

K.E. Green (2002)
With the passing of my Mom on April 6, 2002 came the question, What can I do to help? Shirley Green did
more than "help". She organized the reception, bought the food, set up the reception area, had the food
and tables ready when we arrived from the funeral. Her professionalism showed through without a moment
of hesitation. She was caring of everyone's needs and did a wonderful job that took the stress off of all
the family that was going through a very difficult time that we all must go through at some time. Her "help"
will never be forgotten.
